What is citicoline?

Frontal-lobe scans showed increases in phosphocreatine (+7%), beta-nucleoside triphosphates — largely ATP in the brain — (+14%), and the phosphocreatine-to-inorganic-phosphate ratio (+32%), alongside changes in membrane phospholipids. In plain terms: the brain region most associated with focused attention showed measurably altered energy chemistry after supplementation.

An honest caveat belongs here: Grieb's review also stresses that there is still "no adequate description of the mechanism(s)" that fully connects these biochemical changes to cognitive outcomes. The biochemistry is real; the causal chain is still being mapped.

Does citicoline improve memory?

The best single piece of evidence says yes, modestly, for episodic memory in older adults — with real caveats.

Nakazaki and colleagues (2021) ran a randomized, double-blind, placebo-controlled trial in 100 healthy adults aged 50–85 with age-associated memory impairment — the everyday memory slippage of normal aging, not a diagnosed condition. Participants took 500 mg of citicoline daily with breakfast for 12 weeks.

The results, honestly stated:

  • Episodic memory (the Paired Associates test) improved significantly versus placebo (p = 0.0025).
  • Composite memory — an aggregate across tests — also improved significantly (change score 3.78 vs 0.72 for placebo, p = 0.0052).
  • The trial's primary outcome, a spatial working-memory test, did not reach statistical significance. The wins were on secondary outcomes, which is weaker evidence than a primary-outcome hit.

This trial sits within a broader picture: a 2023 review in Aging and Disease concluded that "oral citicoline intake positively influences memory function in humans who encounter age-related memory impairment also in the absence of any detectable neurological or psychiatric disease."

Disclosure matters when weighing this: the 2021 trial was sponsored by a citicoline manufacturer, and two authors were employees of the sponsor. Industry funding doesn't invalidate a well-run randomized trial, but a skeptical reader should know it's there — and should also know that Europe's food-safety regulator read the same literature less generously (more on that below).

Does citicoline help with focus and attention?

Two placebo-controlled trials in healthy people found improvements in attention at doses of 250–500 mg per day over 28 days.

In McGlade and colleagues' 2012 study, 60 healthy women aged 40–60 took 250 mg citicoline, 500 mg citicoline, or placebo daily for 28 days, then completed the Continuous Performance Test II — a 14-minute sustained-attention task that counts two kinds of mistakes: failing to respond when you should (omission errors, a lapse of focus) and responding when you shouldn't (commission errors, a failure of inhibition).

After 28 days, the 250 mg group averaged 0.90 omission errors versus 6.70 for placebo (p = 0.04) and also made fewer commission errors (p = 0.03). The 500 mg group made significantly fewer commission errors (p = 0.03), with a trend toward fewer omission errors. The authors' summary: citicoline "was associated with improved attentional focus and inhibition" in healthy adults.

A second randomized trial by the same group (2015) tested 250 mg or 500 mg daily against placebo in 75 healthy adolescent males for 28 days, and found improved attention (p = 0.02) and increased psychomotor speed (p = 0.03) in the citicoline groups.

Consistent direction across two trials is encouraging. The same disclosure applies, though: both studies had industry-affiliated co-authors, the samples were small, and each studied a narrow demographic — so the results are suggestive rather than settled.

Does citicoline increase brain energy?

Imaging evidence shows changes in the brain's energy metabolites — but "brain energy" here means biochemistry, not a stimulant buzz.

Two practical notes from the trial protocols: the memory trial dosed with breakfast, and every positive finding came from daily, sustained use — none of these studies measured single-dose effects. What are the limits of the evidence?

A fair reading of the citicoline literature has to include the strongest counter-argument, and it comes from a serious source.

In 2024, the European Food Safety Authority's expert panel evaluated a proposed health claim that citicoline supports memory in middle-aged and older adults. The panel concluded that a cause-and-effect relationship has not been established — primarily because, in its judgment, no convincing mechanism was demonstrated by which supplemental citicoline (beyond what the body already synthesizes) would improve memory in healthy people.

How do you square that with the positive trials above? The honest synthesis: the randomized-trial signal is real but modest, concentrated in secondary outcomes and small samples, much of it industry-funded, and the mechanistic bridge from biochemistry to cognition isn't yet nailed down to a regulator's standard. In the 12-week randomized trial, no adverse events were judged serious; eight mild events were considered possibly product-related across both groups — six with citicoline, two with placebo — and the citicoline-related events were mild and transient. Grieb's review describes the compound as "non-toxic and very well tolerated," with reported side effects "mostly related to digestive disturbances following oral intake" — occasional stomach upset, in practice. The 2023 memory review likewise notes no significant adverse effects and no identified contraindication across decades of use.
