Choline Benefits: What the Evidence Actually Shows
Choline is an essential nutrient that most people have never checked their intake of. Your liver makes a little. The rest has to come from food, and United States survey data suggest that about nine in ten people do not eat the recommended amount. That matters because choline sits underneath three basic jobs: making the neurotransmitter acetylcholine, building the membranes around every cell, and moving fat out of the liver.
This article walks through what choline does, the benefits with human evidence behind them, how much you need, and where to get it.
What is choline?
Choline is a nutrient your body needs but cannot make in sufficient amounts. The liver produces some by methylating a membrane fat called phosphatidylethanolamine, but as a 2025 review puts it, this route "produces limited quantities of choline, insufficient to meet physiological needs". The Institute of Medicine recognised choline as essential in 1998 and set daily intake targets.
Choline is not a vitamin, although it often gets shelved with the B vitamins because it works with folate and vitamin B12 in the same methyl-transfer reactions. It is not a mineral either. It is a small nitrogen-containing compound that the body uses both as a raw material and as a chemical messenger precursor.
What does choline do in the body?
Choline does three jobs, and each one explains part of the deficiency picture.
First, it is the direct precursor of acetylcholine. The review above describes choline as "a precursor for acetylcholine, a key neurotransmitter, important for functions such as memory, muscle control, and mood regulation". Neurons cannot make acetylcholine without it.
Second, choline is built into phosphatidylcholine and sphingomyelin, the phospholipids that form the membrane of every cell. Membrane synthesis consumes most of the choline you eat.
Third, choline is a methyl donor. Its oxidised form, betaine, feeds methyl groups into the cycle that produces S-adenosylmethionine, the compound that methylates DNA and many proteins. This is where choline, folate and vitamin B12 overlap, and why a shortage of one leans harder on the others.
The liver connection follows from the second job. The liver packages fat into lipoproteins for export, and that packaging requires phosphatidylcholine. Run short of choline and fat accumulates in the liver.
What are the benefits of choline?
The clearest benefit of choline is preventing the liver and muscle damage that appears when intake is too low; the evidence for brain and pregnancy benefits is promising but less settled. It helps to grade each claim honestly.
Liver health. This is the strongest evidence, because it comes from controlled human experiments. In a depletion study at the University of North Carolina, 57 healthy adults ate a diet with almost no choline. Within weeks, 77% of the men and 80% of the postmenopausal women developed fatty liver or muscle damage, and the damage resolved when choline was restored. Hospital nutrition provides a second line of proof. Patients fed intravenously for years developed fatty liver on choline-free formulas, and in a placebo-controlled trial adding choline to the feeding solution improved liver density on scans from week four onward, while the placebo group did not. When the choline stopped, the liver fat came back.
Brain function. Here the evidence is observational. In the Framingham Offspring cohort, researchers measured choline intake in 1,391 adults and tested their cognition. Higher intake went with better verbal memory and visual memory scores, and earlier intake went with less white-matter damage on brain scans. The authors are careful to say the design cannot prove cause, and a supplement trial using plain choline bitartrate in healthy young adults found no acute memory gain. So eating enough choline tracks with better memory in middle age, but swallowing extra before an exam has no trial support.
Pregnancy and development. A controlled feeding trial at Cornell randomised women in their third trimester to 480 or 930 mg of choline per day. Averaged across four tests between four months and thirteen months of age, infants of the higher-choline mothers reacted significantly faster, which the authors read as better information processing speed. The trial was small, 24 infants, but it was randomised, double-blind and tightly controlled, and it matches decades of animal work. A 2019 review notes that fewer than one in ten pregnant women reach the recommended intake.
How much choline do you need per day?
Adult men need 550 mg per day and adult women 425 mg, rising in pregnancy and lactation. These are the Institute of Medicine's adequate intake values from 1998:

The upper limit for adults is 3,500 mg per day, set to prevent low blood pressure and fishy body odour at extreme doses.
Most people fall short of the targets. An analysis of national survey data found that only 11.3% of the United States population aged two and over met the adequate intake for choline, with adult men averaging 405 mg and adult women 273 mg per day. Falling short of an intake target is not the same thing as clinical deficiency, but the margin is thin, and requirements vary from person to person more than most nutrients because common gene variants change how much choline the liver can make.
What foods are high in choline?
Eggs and organ meats top the list, with meat and fish behind them, and soy the best of the plant sources. Egg yolk is the most concentrated common source, at about 680 mg per 100 grams. Some measured servings:

As a rule of thumb from the 2019 review, beef, eggs, chicken, fish or pork provide more than 60 mg per 100 g, while nuts, legumes or cruciferous vegetables provide at least 25 mg per 100 g. A day with two eggs, a portion of meat or fish, and some beans or broccoli reaches the target without effort. A day without eggs or meat usually does not. The authors of the survey analysis above concluded that reaching the choline target without eggs or a supplement is extremely difficult, which is worth knowing if you eat a mostly plant-based diet.
What are the signs of low choline?
In the controlled studies, low choline showed up as liver fat and muscle damage, usually without obvious symptoms at first. In the North Carolina depletion study, the marker of muscle damage, creatine phosphokinase, rose more than fourfold on the choline-free diet, and liver fat climbed on imaging. Both reversed when choline came back. There was no pain and no dramatic warning sign, which is what makes the shortfall easy to miss.
Some people are more vulnerable than others. Premenopausal women were partly protected in the depletion study, and the researchers proposed that oestrogen raises the activity of the enzyme that makes choline in the liver. Men and postmenopausal women developed problems at much higher rates. Genes matter too: in the same study, premenopausal women carrying a common variant in the MTHFD1 folate gene were more than 15 times as likely as non-carriers to show signs of choline deficiency. Pregnant and breastfeeding women also need more, because the child is drawing on their supply.
True clinical deficiency is rare in people eating a normal mixed diet. The concern documented in the surveys is the wide gap between typical intake and the target, in a nutrient where controlled studies show organs suffer when intake stays too low.
Should you take a choline supplement?
If your diet reaches the intake target from food, you do not need one; if it does not, a supplement is a reasonable way to close the gap. The form matters more than most supplement decisions, because the forms differ in how much choline they carry and in what the research tested.
Choline bitartrate is the cheapest and is about 41% choline by weight. It covers a dietary shortfall, but the acute memory trials that used it in healthy young adults came back negative.
Citicoline, also called CDP-choline, is about a fifth choline by weight, but it brings cytidine along, and most of the human cognition research on choline compounds used it. We have covered that research in detail in our citicoline benefits article.

Multivitamins rarely help here. Most contain 25 to 50 mg of choline, a small fraction of the daily target, so check the label rather than assume it is covered.

The practical summary: eat eggs, meat, fish or soy most days and you will likely reach the target. If you do not, or you are pregnant, or you want the form the brain research actually used, a supplement that names its choline form and dose is the honest option.

Is choline a B vitamin?
No. Choline is grouped with the B vitamins because it works alongside folate and vitamin B12 in methyl metabolism, but the Institute of Medicine classifies it as an essential nutrient in its own right. Unlike a vitamin, your liver can make some choline. It cannot make enough, which is why a daily intake target exists.
Can you take too much choline?
Yes. The adult upper limit is 3,500 mg per day, set to prevent low blood pressure and a fishy body odour. Both problems come from doses far above what food or a normal supplement provides. A typical diet supplies 270 to 405 mg per day, so most people sit nearer the floor than the ceiling.
Do eggs give you enough choline?
One large egg provides about 125 mg of choline, roughly a quarter of a man's daily target and a third of a woman's. Two eggs plus meat, fish or dairy across the day will usually reach the target. United States survey data show that people who eat eggs are far more likely to reach the recommended intake than people who do not.
Does choline improve memory?
The evidence is mixed and depends on the form and the person. What is the best form of choline in a supplement?
It depends on the goal. Choline bitartrate is the cheapest way to add choline to the diet. Citicoline and alpha-GPC cost more per gram of choline but are the forms used in most of the brain research, and citicoline also supplies cytidine, which cells use to build membrane phospholipids.
